You may print & fill out your form to speed up the intake process during your first visit.
If you do not have the ability to print it prior to coming one will be provided to you by our receptionist,
please come 10-15 minutes before your appointment start time!

Cancellation Policy
We understand that life happens and not all plans go smoothly. We kindly ask that if you need to, please reschedule or cancel at least 12 hours before the beginning of your appointment; so that we can offer the appointment slot to others who may need it. However, if the cancellation is last minute or if you call us after your appointment has started to let us know you are not going to make it, you will be charged the full appointment fee. We thank you for your understanding!
